Proactive MD is actively transforming the legacy on-site employer clinic industry through its next-generation Health and Wellness Centers. We are growing and looking to add to our team an innovative and forward-thinking, bilingual (English/Vietnamese), Patient Advocate for our Health and Wellness center located in Buford, GA.
The mission of the Patient Advocate (PtAd) is to facilitate and ensure continuity in patient care and serve as a conduit of care between health services in the community. The PtAd will strategically engage employees and their dependents before, during, and after visits to health centers in order to ensure they are receiving personalized, informative, and compassionate care. The PTAd role is supported by and reports to the Vice President of Service Delivery. The individual will work closely with the Vice President of Service Delivery, Account Manager, Medical Assistant, Provider, Regional Medical Director, and Client HR Team.
Coordination of Care
The PtAd will coordinate care with external specialists, labs, medical equipment providers, counselors, health classes, etc.
Assist patients in navigating the healthcare system.
Coordinate care when a patient is admitted and discharged from the emergency room or hospital. Ensure the patients Provider is included in the aftercare process.
Visit hospitalized patients to ensure care coordination is directed by their Provider.
Encourage and coordinate follow up visits with Primary Care Providers.
Analyze data and develop and lead outreach campaigns and educational resources. The PtAd is responsible for the execution and success of these campaigns
Represent Proactive MD at meetings and events.
Be knowledgeable about and provide information to patients regarding available community services that could enhance their care.
Plan, coordinate, and lead patient education and wellness events and classes.
Be knowledgeable about the company’s medical benefit plans and serve as a resource to patients in navigating plan types and services available.
Coordinate patient care and treatment plans; provide educational materials/consultation; follow up on patient well-being
Reporting & Analytics
Analyzes data to bring forth information as it pertains to population risk management. The PtAd utilizes this data in collaboration with the Medical Director to develop outreach campaigns and outreach programs accordingly.
Evaluate historical claims data; Develop programs, outreach campaigns, and educational resources that will encourage Primary Care utilization.
Assists with client relations. Generates and provides reports or documents for the Account Manager related to benefits, clinical values, and programs.
Promote the role of the PtAd to prospective clients. This could be done in coordination with Business Development and may include travel to prospective sites.
Responsible for the development of engagement strategies. This will be done in conjunction with the onsite Clinical Teams and Account Managers.
Skills & Qualifications
5+ years of experience working in healthcare setting
Medical or Healthcare social work in similar setting, or case management experience
Master’s or Bachelor’s degree in Social Work or related field
Knowledge of the concepts and principles of case management
Ability to communicate effectively and maintain working relationships with people from diverse backgrounds
Knowledge of the healthcare industry and available patient resources
Knowledge of payors
Ability to prioritize needs and plan work accordingly
Knowledge of HIPPA confidentiality requirements
Preferred: The following are desirable skills and experience
Master’s degree in Social Work
Medical social work experience
Knowledge of third party payer requirements
Knowledge of medical terminology
Bi-lingual in English and Vietnamese
Presentation Skills/Speaking in front of audiences
Creating & tracking plans
The following physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job: While performing the duties of this PAd, the employee is regularly required to talk, see, and hear. The employee frequently is required to stand and; walk for most of the day, and occasionally lift office products and supplies up to 20 pounds.
Position Type and Expected Hours of Work
This is a full-time position, and office hours vary by facility.
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
