Meets all competencies required for registered nurse.
Provide consultation to all clinical areas for wounds, ostomies, incontinence, and actual or potential skin breakdown according to hospital policies and protocols.
Leads the development and monitoring standards, guidelines, and pathways for care delivery of patients with wounds, ostomies, incontinence and actual or potential skin breakdown. Assists in identifying, monitoring and controlling outbreaks of nosocomial infection among patient and hospital personnel.
Makes recommendations to the physician for changes to wound care orders and provides evidence based research as needed.
Provides education for staff and patients regarding ostomy, wound, pressure ulcer prevention, and infection control during orientation and ongoing, through a variety of communication venues.
Assess and document wound assessment data and communicate findings, revise/develop the plan of care and verbalize pertinent information to the health care team.
Coordinates patient care in collaboration with other providers for the population served.
Accurately performs patient assessments and identifies patient needs.
Identifies and initiates appropriate nursing interventions.
Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
Provides care appropriate to condition and age of the patient.
Works cooperatively and actively participates with other members of the interdisciplinary team
Develops the annual hospital wide Infection Control Plan; evaluates the effectiveness of the program and reports findings to leadership and appropriate committees.
Assures systems are in place to report infection surveillance, prevention and control information to:
appropriate staff in the hospital, federal, state and local public health authorities in accordance with law and regulation, and accrediting bodies; other referring or receiving hospitals or organizations needing information regarding the presence of infection in a patient received or transferred
Identifies potential sources of nosocomial infection and implements actions to eliminate the source; and evaluates infection data to distinguish between nosocomial and community-acquired infection.
Maintains surveillance in order to collect, analyze and trend data; provides accurate and timely reports to the Director of Continuous Quality and Performance Improvement, leadership and appropriate committees. Incorporates data and information to continuously improve care and practice to enhance clinical outcomes.
Assures the hospital implements infection prevention and control activities to reduce the risk of infections associated with medical equipment, devices and supplies.
Demonstrates ability to work collaboratively and lead work groups, committees and individual staff in organized and efficient manor to accomplish expected goals and outcomes.
Collaborates with Employee Health/Human Resources in the prevention and surveillance of infections among hospital personnel; includes interventions for immunization and/or treatment to staff and licensed independent practitioners.
Communicates and collaborates with the Laboratory in order to identify significant laboratory information.
Communicates the mission, ethics and goals of the hospital, as well as the focus statement of the department.
Maintains documentation to support compliance in regulatory requirements, including all state, federal and Joint Commission regulations for initial orientation competencies, annual training and competencies as well as infection control requirements.
Communicates employee observations and suggestions related to opportunities for improvement and staff educational needs.
Maintains positive relationships with all customers.
Complies with regulatory and accreditation standards.

Maintains ethical practices and HIPAA requirements.
Ensures compliance with policies and procedures regarding department operations, fire, safety, and infection control. Maintains medical staff rules/requirements, as well as regulatory body standards.
Promotes rehabilitation nursing philosophy and scope of practice.
Actively participates in team conferences.
Performs timely and appropriate documentation relating to medical necessity in the medical record.
Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
Current licensure as a Registered Professional Nurse, issued by the State Board of Nursing.
Certified in Wound/Ostomy Care and/or a minimum of two years' experience as a wound care nurse
Four to six years' experience preferred.
Current Basic Cardiac Life Support
Demonstrated ability to plan, coordinate, evaluate and direct activities of clinical personnel; effective oral and written communication skills, in English with additional languages preferred; proficiency in basic computer skills required.
Qualified Infection Control Practitioner, Certification Board for Infection Control (CBIC), preferred.
