Valley Hospital is seeking a dynamic and talented Lead Utilization Management Coordinator/Case Manager to join our team!

The Lead UM/Case Manager contacts external case managers/managed care organizations for certification and recertification of insurance benefits throughout the patient's stay, and assists the treatment team in understanding the insurance company's requirements for continued stay and discharge planning. The Lead is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team. The Lead advocates for the patient's access to services during treatment team meetings and through individual physician contact. The Lead Chairs treatment team meetings and continued stay reviews as indicated. The Lead will also carry a caseload, will meet with patients daily, be responsible for completing assessments, family sessions, staffing's, and coordinating with outpatient providers. Continue to have safe discharge placement, medication, and outpatient providers. Patients in need of behavioral health services can get referral for SMI services-Seriously Mentally Ill and if they have medical can be referred to ALTCS-Arizona Long Term Care.
